Dear Raby Families,

 

We are excited to welcome our students back IN PERSON EVERY DAY for the 2021 - 2022 school year.  Please mark your calendars for registration.  We are asking that all families attend this event to make sure students are prepared for the first day of school.

 

REGISTRATION WILL TAKE PLACE AT RABY

9th grade only-  Thursday, Aug. 5th   10am  Required-This is Freshman Orientation

10th grade only-  Thursday, Aug. 5th  12pm  Required-This is Sophomore Orientation

11th – 12th Grade Monday, August 9th    1pm -4pm    Required

11th - 12th Grade Tuesday, August 10th   10am -1pm   Required (if you NOT attend on 8/9)

 

PLEASE ATTEND ON YOUR SCHEDULED DATE

 

Registration will take place in the Cafeteria, Door 4 on Central Park. To ensure the safety of our Raby family we ask that you please adhere to the following guidelines:

Face coverings are required

We ask that each student bring no more than 2 parents/guardians/guest

We ask that each family please practice physical distancing of 6 feet from other families, if there is a line

Please ensure all enclosed forms are completed prior to attending registration

Please use hand sanitizer when at the registration table

If you are sick on these given dates, please stay home

 

In preparation for the first day of school, all students will need the following:

 

  1. Proof of current address (Documents can include any TWO of the documents.  No personal mail.)

Current utility bill

Illinois driver’s license or State of Illinois identification card

Deed

Employee identification number

MediPlan/Medicaid card

Court documents

Illinois Department of Public Aid Card  

Stamped United States Post Office change of address form

Illinois state aid check/social security check

  1. Updated Emergency Contact Information (enclosed)
  2. Student Medical Information for the 2021-2022 School Year (enclosed)
  3. School Messaging Consent Form (enclosed)
  4. Media Consent Form (enclosed)
  5. 2021-2022 Family Income Information Form (enclosed) 

In addition, all incoming freshman should also bring

Copy of Student’s Birth Certificate

Copy of 8th grade diploma

Proof of Physical Examination

Immunization Requirements (All incoming freshmen MUST get shots to be in compliance)

Diphtheria, Pertussis (Whooping Cough), Tetanus (DTP/DTaP & Tdap)  

Polio 

Measles, Mumps, Rubella (MMR)

Hepatitis B 

Varicella (Chicken Pox) 

Haemophilus Influenzae, Type B (HIB)

Pneumococcal Conjugate (PCV)

Meningitis Conjugate (MCV4)

 

During registration, parents/guardians will be able to pay school fees (to cover instructional materials and the usage of school property such as locks or laboratory equipment) as follows:

Freshmen                                $150 (this includes the required PE uniform)

Sophomores                            $100

Juniors                                     $100

Seniors                                    $100

Low Incidence Program         $100

                             

Fees should be paid during registration.  All school fees must be paid by Friday, October 1, 2021. 

Cash, money orders, credit/debit cards accepted in person - NO CHECKS however, you may pay online via debit or credit card at alraby.org (scroll down to Epay).

 

Families can request a fee waiver to waive a portion of the school fees.  Please contact the Main Office to receive a fee waiver.  Fee waivers must be submitted to the Main Office by October 1, 2021.

 

If your child is OUT of medical compliance, they must go to the doctor before enrollment will be completed.  If you have a question regarding your child’s medical compliance status, please contact Ms. Davis at JSDavis2@cps.edu or 773-534-8903.
